Before walking age clamshell orthosis to prevent fracture 
Initial surgical mx - mechanical alignment and intramedullay fixation 
Resection of the pseudarthrosis,
primary shortening, and supplemental bone grafting

Amputation for those with a history of multiple
failed surgical procedures, and stiffness and decreased
function of a limb that would be more useful after an amputation
and fitting with a prosthesis
